Forms & Directions to Your Appointment

The Personal History Form (or Follow-Up Form) must be filled out in advanced. It can be mailed to you, or you may download it with the link below. Please be aware that the use of this form is specific to my healing work. Other counselors or acupuncturists will use their own form. You do not need to mail or send it on to me in advance of our appointment. I prefer that you bring it with you to the initial meeting.
